CPSL Match Report Form

The Referee assigned to any Charlotte Premier Soccer League match is required to fill out this online Match Report and complete the same report in Arbiter. If there has been a player, coach or spectator sent off during any CPSL match, the USSF Supplemental Send-off Report is also required. The link to that online form is located in the header of the Arbiter Match Report.

NEW IN 2009 - 2010: The referee crew is required to check Player Passes and Team Rosters prior to every match and require a valid NCASA Player Pass for every player. Immediately following the checking of Player Passes against the Team Roster, Player Passes are to be returned to each team regardless if a plaer is sent off and shown a red card during the match. Each team and the League is responsible for administering Player Pass management. The referee or assistant referee must collect Player Passes of players who arrive late, but must return these Player Passes to the team as soon as they are checked with the Team Roster and at a convenient time, such as during the half-time.

If the referee failed to bring a blank USSF Match Report Form with him or her to the match, the referee should print a copy of this report prior to submitting the form. Remember: Two online Match Reports are required for every CPSL match -- this online form and the Match Report in Arbiter.

Where HT/VT is Home Team or Visiting Team; # is for jersey number; Last name and first name using comma; CODE is either UB, PI; DT; DR; L, E, or FRD; mm is the minute in the match 0-90; and Reason is one or two word description. Note: each category is separated by a semi-colon. Use a new line for each caution issued.
